Your First Visit

Welcome to Our Practice

As a first-time patient, we want you to be completely comfortable before you even arrive at our office. We'll call you one business day prior to your appointment to confirm and answer any questions.

Here's what to bring to your first appointment:

  • Government-issued photo ID
  • Insurance card and insurance information
  • Completed new patient paperwork (download below)
  • Current list of medications and allergies

Cancellation Policy: If you need to reschedule or cancel, please provide at least 24 hours' notice so we can fill your appointment time with another patient who needs care.

Have questions about your first visit? We've answered the most common ones below.

Please bring a government-issued photo ID, your insurance card and insurance information, a current list of your medications and allergies, and your completed new patient paperwork. You can download the paperwork above and fill it out before your arrival to save time.
We'll call you one business day before your appointment to confirm and answer any questions. During your visit, you'll receive a personalized comprehensive exam with Dr. Heather, digital X-rays, and a routine cleaning if clinically appropriate. We'll create a customized treatment plan and provide referrals to specialists if needed.
We accept cash, Visa, Mastercard, and CareCredit (a healthcare financing credit card). Copays and deductibles are due at the time of service. Please note that we do not accept personal checks.
If you need to cancel or reschedule your appointment, please provide at least 24 hours' notice. This allows us to offer your appointment time to another patient in need of care. We understand that emergencies happen and will do our best to accommodate you if you need to reschedule on short notice.
Coverage & Billing

Insurance Information

We work with most major dental insurance plans and will file claims on your behalf. Because plan benefits vary widely, we recommend calling our office before your appointment so we can verify your specific coverage and give you a clear estimate of any out-of-pocket costs.

In-Network & Out-of-Network

We are in-network with many plans and will gladly see patients with out-of-network benefits as well. Call us at 229-242-2449 and we'll look up your plan before your visit.

Copays & Deductibles

Your copay and any deductible balance are due at the time of service. We will provide a good-faith estimate of costs before treatment begins whenever possible.

No Insurance? No Problem.

We accept cash, Visa, Mastercard, and CareCredit financing. CareCredit offers flexible monthly payment plans — ask our team for details or apply at carecredit.com.
